Abstract: Technology has integrated into the health sector to improve patient experience and satisfaction. Electronic health as known as E-health has become a popular medium of data collection in the health sector as it allows valuable data to be gathered within a short period for various purposes. This project aims to develop a Healthcare Survey Management System (HSMS) that ease the health sector in collecting, analysing and managing patients’ records. The system consists of two components, a Web-based Survey Management Application and an Android Mobile Application which cater to two audience groups: healthcare professionals and patients. The development integrated development environments (IDE) used for this project are Eclipse and Android Studio. JSTL tags are used on JSP to build the dynamic web system while Java programming language is used for Android mobile application development. This report presents supportive information that describes the development process of HSMS. It consists of HSMS design methodology, overall description, technical specification, requirement specification, explanation on the design and implementation of the interfaces and features of the system. Several recommendations and suggestions are proposed for future enhancement.
