Abstract: With the rise of aging population and demographic diversification, the medical needs of individual patients become increasingly diversified. Patients, especially those with multiple conditions, demand healthcare services that are tailored to their specific medical as well as financial conditions. However, the current medical system is primarily designed from the healthcare providers’ point of view, aiming to achieve maximum efficiency. More specifically, the healthcare processes have been designed to maximize the utilization of medical resources, which are insufficient and ineffective in addressing individual patients’ needs.
